Player Registration is Now Open! Deadline for Mites/U8 is September 30th

Required Forms:
What forms do I need this year?
- USAH Consent To Treat Form: A new, original form is required every year for every player. A completed and signed copy MUST be received by MYHA before a player can take the ice.
- GOV'T ISSUED BIRTH CERTIFICATE: Players MUST have a birth certificate on file with MYHA. (New players to Mahtomedi or those who have not submitted a birth certificate in the past are required to do so). A copy is acceptable, it does not need to be certified. *This must be a government issued birth certificate - hospital issued copies will not be accepted.
*ALL required forms MUST be received before players will be allowed on the ice
