MYHA Registration information

2009-2010 Registration Materials: Past Due for Travel Levels. Initiation Levels due 9/19.

For all travel levels (Squirt, Peewee, Bantam, Jr Gold/U16, U10 Girls, U12 Girls) Registration Materials were due 8/29.  Please get in late materials to Melanie ASAP!

For all initiation levels (Mini-Mite, Mite, U8) - Materials are due by 9/19.

 

2009-2010 Registration Information and Materials

The 2009-2010 hockey season is fast approaching. Again, MYHA is requiring all players and coaches to register on-line with USA hockey (http://www.usahockeyregistration.com). (Link is below)

The USA Hockey season goes from September 1, 2009 through August 31, 2010.
 
To register with USA hockey:
  1. Go to the USA Hockey website www.usahockeyregistration.com (see below)and follow the prompts. 
  2. Choose member type ice player or coach
  3. Must be 18 years of age to process registration, check the box that acknowledges that you are 18 or older.
  4. Select the correct season: 2009-2010
  5. Follow the prompts – Fill in all data correctly. Please give your child’s legal name as listed on his/her birth certificate. DO NOT USE NICKNAMES.
  6. Process payment – Visa, Master Card, Discover or American Express (credit or debit) a visa gift card will also work.
  7. You must print your confirmation page at this time. It will also be emailed to you immediately after the registration is completed. This MUST be turned in with your registration paperwork. Registering with USA hockey replaces the Waiver of Liability form.
 
USA HOCKEY FEE
 
Each participate will pay the USA hockey fee of $30 and MN hockey fee of $10 if age 7 or older. The USA hockey fee and MN fee is waived for age 6 (birth year 2003) and younger. The registration must still be completed to get the confirmation number. *Coaches will be refunded the above fee
 
Registration forms must be mailed to: MYHA Registrar, P.O. Box 604, Willernie, MN 55090. Forms must be postmarked by August 29th or a late fee will incur.
 
You will need to submit the following 4 items to complete the registration process. Please note: New this year: ALL 4 items must be to Melanie for your child to begin the season:
 
  1. 2009-2010 registration form (see tab below)
  2. Consent to treat (see tab below)
  3. Printed copy of USA Hockey player registration with confirmation number (Must register with USA hockey online to receive this number). (details above-tab below)
  4. Birth certificate – new members only
 
Please help spread the word and forward this email to families residing in district #832 that may be thinking of playing hockey!
ALSO NEW THIS YEAR: Any families who have players attending school within the 832 school district will play for Mahtomedi Hockey. This is for Public and Private schools. Please let any families that live outside of district but their children attend school in district know about our registration.

INTERESTED IN COACHING LEVELS?
Please see COACHES CORNER tab on the left.
The following 4 items are needed:
 
  1. Application – fill out and send email to Brian Hamernick @ bhamernick@comcast.net
  1. The following 3 items need to be completed and handed in to Melanie at walk-in OR mailed in to the PO Box:
    1. USA hockey Confirmation number (page can be printed after you register). 
    2. Background screening form (filled out completely including SS#)
    3. Updated copy of coaches card- front and back sides. Please make sure Melanie has the most current record of your coaching level certification.

USA Hockey Ice Player and Coach Registration-MUST DO FOR REGISTRATION

This is the link to USA Hockey. All players and coaches must register with USA Hockey. Your confirmation page must be submited with your registration to play and with your paperwork to coach.
